ABOUT THE PARTNERSHIP FOR FAMILY HEALTH
MISSION
The mission of the Partnership for Family Health (PFFH) is to provide comprehensive, multidisciplinary, family-centered medical, dental, mental health care and social services to HIV infected and affected families including women, infants, children, youth, male caregivers and their families. PFFH further strives to educate the community and prevent HIV transmission in these target groups.
HISTORY
The Partnership for Family Health, formerly the Northern Manhattan Women and Children HIV Project, began in 1988 as one of the original programs aimed at serving HIV infected women and their families. It has been funded by Title IV of the Ryan White CARE Act since the CARE Act was created in 1990. Since its inception, six sites have successfully collaborated to provide a comprehensive, family-centered system of care.
MODEL OF CARE
The Partnership for Family Health offers a family-centered, comprehensive, multidisciplinary model of care where women, children and their families may receive all of their care through a 'one-stop-shopping' arrangement. Outpatient care for all family members is provided at one location by a team of providers. Children and adolescents receive primary and specialty care, as do their mothers and siblings. This seamless approach reduces barriers to care and maintains continuity.
